Checking Students' Vital Signs of Learning

The purpose of this module is to give learners an opportunity to indicate areas where their learning experiences have been easy and rewarding, and where they have encountered difficulty or frustration. (Recommended for students ages 5th – 12th grades.)

Activity Steps for Module B

These activities can be done by individual students with a parent or teacher, if desired.
Encourage students to take the survey seriously and honestly.

As with the medical “vital signs,” it is important for students, parents, and teachers to routinely recognize when certain “vital signs” of learning appear to be problematic. Honest responses to the questions in the Vital Signs of Learning Survey will help create a profile of a student’s current strengths and needs in core subjects.

Keep in mind, there are no “right” or “wrong” responses - unless a student is not answering honestly, based on their personal experiences in school. The whole purpose of the survey is to help students, parents, and teachers identify areas of strength that need to be supported and areas of possible need that might be causing academic frustration.

Survey participants will receive a report of their results (strengths and needs) that can then be used for a personal interview session with a certified teacher to build a set of recommendations that students and parents can share with the student’s teachers.

  1. Take the survey and submit it. Be sure to indicate an email address and phone number for follow-up reports. It takes on average about 20-25 minutes to answer the 20 questions.
  2. When you get results, you will have the opportunity to arrange for a consultation with an experienced teacher to interpret your results and jointly develop recommendations to share with your current teachers.
  3. Share your results and experience in The Forum
